The IDHSAA 2024-25 physical forms are needed for freshmen and juniors. And the interim questionnaire is needed for Sophomores and Seniors prior to tryouts. Each student need documents uploaded to www.registermyathlete.com. Pictures on a phone can work if they are clear.
